An award winner must first be nominated through the Superstar Foundation website. Please click on the appropriate button below if you are a manager or supervisor and would like to nominate a member of your team OR if you are a Superstar and would like your manager or supervisor to nominate you.
Only direct service staff who provide services to the target population of a given organization are eligible for a Superstar Award.
The 10 question nomination form will take approximately 1 hour to complete. You may stop at any time during the nomination process and return to complete the form at a later date. There are four components of the nomination form:
Please note that some questions are posed to the nominator; others such as the organizational theory of change and its application are to be answered from the perspective of the nominee. Successful applications have been submitted without the knowledge of the nominee and the best nominations ensure that their perspective is effectively communicated.
